Membership Forms

Memberships and waivers are valid through the end of the calendar year.  Each must be renewed after 12/31.  Please do not email or fax your waiver.  Waivers can be mailed to UPA, 4730 Table Mesa Dr, Ste I-200C, Boulder CO 80305 or members may sign waivers online through their personal accounts.

Additional Registration Forms

Medical Authorization Forms are valid for one year from the date they are signed. 

• Medical Authorization Form (for minors under 18 yrs. of age)

Chaperone Forms are valid through the end of the calendar year.  Each must be renewed after 12/31.
